Epitaph:

Here lies Kethoth, resurrected as a dog only for that dog to play catch with a lightning wand. Left as nothing but ashes twice

Obituary:

Kethoth was blasted by a death ray from Bill Gates right as he unlocked the secret to defeating him. Before he could inform his friends about the weakness, Bill toasted him. 
Left as nothing but ash, his friends decided to stuff a nearby dog with the ashes and try to resurrect the dog. The dog springs to life as another adventurer is throwing things he is finding from a chest. He throws a wand of lightning. The dog thinking this is a game, chases after the wand and gets electrocuted, turning into ash.
