Fell down the center of a bell tower, crushed by the bell.

Obituary:

Baziran was a dragonborn wizard that specialized in evocation spells. He was played for a few sessions, the final of which took place on a bell tower. Cornered by undead monstrosities at the top of the tower, he conjured up a gust of wind to knock them into the center. The gust also knocked loose the bell, which crushed him and the undead at the bottom of the tower.
